Step 1: Fill in your personal information.
This includes your first name, last name, and email address (if you do not have an email address, include your mailing address instead.

Step 2: Compose your message.
Write a brief description of your photo and how PreserVision^® eye vitamins are helping you preserve your vision.

Step 3: Find your photo.
Click the Browse button to locate your photo on your computer.

Step 4: Submit.
Once you've completed all the steps, cick Submit to upload your photo.
